What to bring

Your lab order
We accept order forms from all labs and clinics, including Quest and Labcorp, and printed orders from your medical professional.

Proof of identity
Driver's license or passport.

Proof of insurance
Your insurance card.
Patient portal
Get your lab results.
- Open the Patient Portal using the button below.
- Select “Log In” if you already have an account. New patients: wait at least 48 hours after your first sample collection, then select “Sign Up.”
- Once signed in, access your available lab results in the portal.
Need help signing in? Use “Forgot User ID Or Password?” on the portal, or email outreach@valleydiagnostics.org for portal assistance. Do not include medical information in your email.
